Onychomycosis - Its differentiation against psoriasis of the nails by means of capillary microscopy

Resumo

If one does not find fungi in diseased fingernails, sometimes it is hard to decide whether we have overlooked the pathogen, or whether one is dealing with psoriatic nails, since mycosis of the nails resembles both conditions. Capillary microscopical investigations of the nailfolds of the fingers demonstrated to us that we may discover in 3/4 of suspected cases, a characteristic formation of “cotton-balls” of the capillaries. In patients suffering from onychomycosis we find remarkable twisting and torsions of the limbs of the capillaries as well as a tendency of the bloodstream to manifest granular flow, with stasis in the vessels. In using the capillary microscope for this limited purpose we have found a means of differentiating onychomycosis from psoriasis of the nails, with high probability.
